Weaker Netherworld Creatures were unable to descend into reality, but more powerful ones possibly could.

However, the barrier between the Nether Realm and reality was by no means easy to penetrate. Moreover, just like when an otherworldly being enters the Nether Realm, a creature from the Nether Realm trying to enter the real world would appear at a random location—unless someone anchored a coordinate for it.
