The Enhanced Doctor

Chapter 1349: Visceral Edema

The next morning, Liu Banxia got up early. The patient from last night had been in rescue until dawn, although it was Li Mingwu’s team handling it.

It was pretty much as everyone anticipated, the patient developed a pulmonary embolism. However, medication couldn’t control it as the patient had wounds from a fascial decompression incision and fractures, so surgery was the only option.

"The cut you made yesterday was pretty good, but weren’t you afraid of hurting the patient?" Wang Chao leaned in next to Liu Banxia.

"No, the patient was wearing quite a few clothes, and with the infiltration of concrete, cutting through the protective clothing wasn’t easy," Liu Banxia said with a smile.

"Today, the main responsibilities of admission and surgery will have to be taken by you guys. My arm is quite sore. I need a day’s recovery; there’s still a surgery in the inpatient department tomorrow," Banxia said.

"Oh, you’re really busy. But now the patient’s family has finally been contacted and is on their way over," Wang Chao said.

"At least the life is saved; next, it’s just about getting the body to adjust and recover slowly," Liu Banxia said.

"If the rescue had been any quicker yesterday, it would have been better. Breaking through all that concrete wasted too much time. If only there were machinery with smaller vibrations and faster cutting, as concrete isn’t easy to cut," he added.

"Oh, Director Liu, our rescue was already quite swift yesterday, where do we find more stable and faster methods?" Wang Chao said with a bitter smile.

"If the patient hadn’t had fractured ribs, it might have been a bit better. Anyway, I think the patient’s endurance is already quite impressive, given they had so many incisions and broken ribs. Even the slightest vibration would be very painful," Banxia said.

"But the casualty situation in yesterday’s accident was serious. The initial accident is said to have been caused by a commercial concrete mixer truck. There’s no conclusion yet, just hearsay," he added.

Liu Banxia nodded, "Let’s leave it to the relevant departments to handle. How are the patients who were sent here yesterday?"

"They’re all doing well currently. Two went into the ICU; the rest are being treated in the wards. The city hospital and the first hospital have more severe cases; they were busy all night too," Wang Chao said.

At this point, Liu Yiqing and the others yawned as they walked over. Last night’s busyness was a heavy burden on them too.

For now, Liu Banxia’s physical condition is much better than theirs.

After adjusting his mindset, Liu Banxia led everyone to start the rounds.

The patients sent in yesterday were distributed between the first and second departments, after all, they couldn’t all be placed in one department. Especially since the reception frequency of the six little ones was relatively good, many patients ended up in the second department.

Currently, the number of inpatients in the second department is quite acceptable; as a newly opened department, it wasn’t easy.

Going through this round took nearly an hour.

Still, it was Liu Banxia’s characteristic way of making rounds, with very little inquiry into the condition. Even if some patients inquired about their conditions, their primary care doctors provided direct answers.

The person responsible handles it. Those conditions are what the primary care doctor should master, and it must be left to them.

After this round, Liu Banxia also gained a comprehensive understanding of the patients’ conditions in the department. Especially those few who came in from the car accident yesterday; they can be considered new patients.

"Alright, there’s nothing up in the department. Who’s on duty upstairs today?" Liu Banxia asked.

Liu Yiqing and Huang Bo raised their hands.

"Okay, you two stay, the remaining three come with me to the first floor," Liu Banxia nodded.

As for duty, he didn’t set any rigid regulations; letting them sort it out themselves. It’s not a big deal, and they wouldn’t slack off because of it.

"You came down just in time. I just got a call; a patient with abdominal injuries will be sent over. The abdomen is lacerated, the intestines are exposed. The head was also hit seriously," Qi Wentai quickly said when he saw him stepping out of the elevator.

"Goodness, why is it so dangerous? Miao Rui, Li Hao, prepare for the reception," Liu Banxia said.

"It was discovered by a passerby this morning, initially thought to be a drunken patient. Only upon closer look, they saw a lot of blood underneath. The police suspect it might be involved in a fight," Qi Wentai said.

"The situation is like this—know how to perform the rescue?" Liu Banxia asked.

The two nodded and quickly donned surgical gowns and prepared blood bags.

Although the patient’s situation hadn’t been seen yet, just hearing Qi Wentai’s description made it clear how dangerous it was.

Just as they finished preparing and reached the entrance, the ambulance and patrol car arrived simultaneously.

"The patient is unconscious, blood pressure 90/60, abdominal laceration, exposed intestines, discolored. High fever, fluid replenishment and oxygen administered on the ambulance," said the paramedic escorting the patient.

"Rescue Room One," Miao Rui shouted.

"Have you contacted his family? Given his condition, the chances of a successful rescue are slim," Liu Banxia said, looking at the police officer.

"We’re still trying to contact him, haven’t found his phone yet, and we’re also conducting interviews at nearby bars," the police officer said.

"Damn, a brawl after drinking? If he had been sent to the hospital in time, it might not have been this serious," Liu Banxia frowned and said.

"He’s certainly been injured for a while, the intestinal tract has necrosis, and there might be sepsis, this is troublesome. Please try to find him as quickly as possible; if luck isn’t on our side, we may not even get to say goodbye."

After saying this, Liu Banxia rushed to the emergency room.

"Teacher Liu, there’s a large amount of effusion in the chest cavity," Li Hao said, holding an ultrasound scan.

Liu Banxia nodded, "Continue the resuscitation as per your procedures, I’ll supervise."

"Chest tube insertion, rapid blood typing, blood biochemistry, blood culture, urine culture, and book the operating room," Li Hao said.

"How do you plan to resuscitate?" Liu Banxia asked, looking at Li Hao, who was inserting a tube.

"First provide fluid resuscitation, blood transfusion, and correct electrolyte imbalances. However, the patient’s core temperature isn’t very high, showing some hypothermia," Miao Rui said.

"We don’t know the main bacteria causing the sepsis yet; it might be contamination of the wound, or intestinal necrosis. Start with third-generation cephalosporin for antibacterial treatment."

"What about the head wound?" Liu Banxia continued to ask.

"Just checked the patient’s pupils are equal; we plan to redo the CT scan after the intestinal surgery. We finished intubation, and quite a bit of fluid drainage," Li Hao responded this time.

"Good, proceed this way," Liu Banxia nodded.

"Sepsis is currently the most serious issue for the patient, we probably won’t have much time for resuscitation. Be prepared for rapid abdominal flushing and examination during the surgery, finish the operation as quickly as possible."

"Got it," Miao Rui replied.

Liu Banxia walked out and went over to the police officers waiting outside, "Sepsis, intestinal necrosis, we can only operate to remove the necrosed intestines first and then redo the head CT scan."

"How are things on your end? Any news? This young man is in quite a grave situation. He was out in the cold for quite some time; the infection is also quite severe."

The police officer shook his head, "No news yet, this is the time bar owners rest, and it takes time to find people."

"Young people these days really dare to get into fights. They pull knives at the slightest disagreement; I don’t know where they get the courage from. Let’s try our best, you handle the treatment first."

"Probably jealousy or some kind of rivalry, then add some alcohol, and they care even less," Liu Banxia said.

"Teacher Liu, we’re done here, we’re taking the patient to the operating room," at this moment, Li Hao and Miao Rui pushed the patient out.

"Okay, I’ll head up immediately," Liu Banxia nodded.

Thought today would be a bit lighter, never expected such a severe case to come in. No idea if he’s a college student; can’t even confirm his identity right now.

By the time he arrived at the operating room, Li Hao and Miao Rui had already started. Li Hao was leading the surgery since he’s been focusing on intestinal surgeries recently.

"How much of the intestine is necrotic?" Liu Banxia asked.

"About thirty centimeters, the blood pressure and blood oxygen levels have improved after fluid and blood resuscitation," Miao Rui said.

"What about other abdominal organs?" Liu Banxia continued to ask.

"Pretty lucky that no other organs are damaged. Seems the abdomen was cut open and the intestines were exposed outside for too long," Li Hao said.

"Alright, speed up the surgery, do it as fast as you can," Liu Banxia said.

Li Hao was puzzled for a moment, not knowing why Liu Banxia was rushing, but figured it wasn’t just for the head CT scan.

However, the surgery isn’t very difficult, just a straightforward intestinal resection and end-to-end anastomosis. The only downside is that the incision isn’t in a very convenient spot, but for the patient’s sake, no additional incisions were made.

While performing the intestinal resection and anastomosis, Li Hao finally understood why Liu Banxia was rushing.

"Teacher Liu, the patient’s abdominal organs are showing some degree of edema," Li Hao said.

"So, keep pushing, there’s no way of closing the abdomen, after suturing, wrap it with cling film, send to ICU, and close it when the swelling subsides. It’ll be a tough battle in the ICU, I don’t have a good feeling about this patient," Liu Banxia said.

The atmosphere in the operating room turned somber because everyone knew Liu Banxia was right. The patient had been delayed too long outside.

Significant blood loss, intestinal necrosis, sepsis, hypothermia, each of these is dangerous, let alone all combined to torment someone.

Li Hao truly gave it his all, carefully washing the abdominal cavity after suturing the intestines.

The patient had been exposed too long outside, and now with organ edema too, this step had to be especially thorough.
